What many allergy sufferers don’t realize is that some of these excipients may be derived from nuts or manufactured on equipment that processes nut-based products. Where the Risk Lies

One major concern is cross-contamination. While medications themselves may not list “peanut” or “tree nut” as an ingredient, they can be produced in facilities that also handle nut-based excipients. In countries like the U.S., unlike food products, medications are not required to label for possible cross-contact with allergens. This means a person could unknowingly take a pill that was exposed to a nut allergen during manufacturing.

Certain excipients may also raise red flags. For example, magnesium stearate, a common pill binder, can be derived from vegetable, animal, or even nut oils, including cottonseed and palm, which pose a theoretical risk if processed with nut-derived oils. Additionally, soft gel capsules often contain oils like soybean or coconut oil, which may or may not be safe depending on individual allergies.

Even pharmacists may not have access to detailed sourcing information. A patient might receive a generic version of a medication without warning that the formulation differs from the brand-name version they tolerated in the past.

  3. Contact manufacturers: When in doubt, contact the drug company directly to ask about the source of excipients and any risk of cross-contamination.  We know that this is not always a straightforward exercise given contract manufacturing and various subsidiary companies, but it's worth a try.

  4. Carry emergency medication: Depending on the severity of your allergies, always carry an epinephrine auto-injector in case of accidental exposure.

  5. Stick with safe options: Once you find a brand that is truly allergen friendly that works with you, stick with it as a safe option to avoid exposure.
